VACCINE STATUS CERTIFICATES (PASSPORTS)
Please do not contact the practice regarding proof of your vaccine status. Please sign up for the NHSapp instead. You can do this via your mobile device or by visiting by clicking HERE.

COVID VACCINATION QUERIES
If you are a registered patient with us and are aged 50 or over, we are please to announce the launch of a new telephone service that patients can use if they have any queries regarding their COVID vaccinations. This includes technical problems such as medical records not matching jabs recieved, difficulties with booking appointments or overdue second jabs. The line will be available Monday to Friday 10am -3pm, starting on 10/05/2021. If you are under 50 years of age, please contact the National booking service. Click here

COVID-19 Vaccinations

We have now started booking appointments for patients at our hubs located at Bingfield St and Hanley Road. We will be inviting patients as per strict directions from Public Health England.

Then the vaccine will also be offered in age order to:

• those aged over 80 years - Offered

• those aged over 75 years - Offered

• those aged over 70 years - Offered

• adults on the NHS shielded patient list - Offered

• those aged over 65 years - Offered

• adults under 65 years with long term conditions (see list). - Offered

Those aged 50-64 years - Offered

Under 50 - Please contact the 119 service in the first instance or the national booking service. Booking are offered based on age brackets not dictated by the practice.

DO NOT CONTACT US REFERENCE YOUR SECOND DOSE - WE WILL CONTACT YOU WHEN YOU ARE DUE, EITHER VIA TXT OR STAFF TELEPHONING YOU.

Please do not call the pracitce. We will send a txt alert or call the relevant cohort of patients at the appropriate time. Please note the Practice will not be directly offering COVID-19 Vaccinations at Ritchie Street at this time.

Please also note that we have no influence over which vaccination is on offer at the sites and when the stock arrives.

Do not call the sites as they will sign post you back to us. Please also note, you will be advised by the vaccinator when your second dose is due. Please arrive on time for your appoointment and be prepared to wait outside.

TESTING for COVID19

We are not routinely offering patients tests for COVID19. Please contact the 119 service for a site near you.

Excemptions for Wearing Face Masks in public

Please wear a facemask if possible, however if for whatever reason you are unable to do this, Please click here for further guidance

Sickness Certificates for Coronavirus/COVID 19

If you have spoken to a clinician, you can download a sickness certificate at the following website: https://111.nhs.uk/isolation-note/ or you can download a self-certificate by clicking here. Please note we will not be issuing any MED3 Fit (Sick) notes at this time.
